If you would like to donate artifacts to the
museum please use the donation form.

Please sign on the donor line and print your
name and address in the spaces provided.

Mail the completed form with your donation.

You will receive a copy back in the mail.

The donation form is provided in .pdf format.

Adobe Acrobat Reader is required to be able to view and print pdf files.
